Registration of the Application of BAPMI's Mediation

The disputing parties that need to settle their dispute through BAPMI's Mediation shall submit a written application to BAPMI, attn. the Chairman, and addressed to BAPMI's office. Both parties, not only by one party, must submit the application together with their agreement that such dispute will settle to BAPMI's Mediation.

Content of Application
The parties that will submit the application for Mediation shall specify all of the following information in the application:

1. agreement of the parties to resolve their problem through a Mediation in BAPMI;
2. a detailed description of the problem;
3. the relevant agreements/documents to be submitted by each of the parties;
4. the name of the Mediator proposed by each of the parties, if any;
5. express statement that the parties will be bound by, will abide by and will carry out each and every stipulation in the settlement agreement;
6. payment receipt of registration cost.

Verification by BAPMI's Management
BAPMI's Management verifies:

1. does the application meets all of the administrative requirements;
2. do the parties have an agreement that such dispute will be resolved by BAPMI's Mediation;
3. is the dispute submitted to BAPMI related to the civil dispute on capital market activity; and
4. do the parties have already paid the Registration Cost.

If the application fulfilled all of requirements as mentioned above, BAPMI's Management will notifies the both parties within a period of 14 work days after registration that the application are accepted and will be further processed.
